UIL/TAPPS Incident Reports

Excerpt from the UIL Constitution and Contest Rules Section 1204 paragraph (o)(4)(C):

Officials must report all ejections of coaches, severe verbal and physical abuse, and any major disruptions that occur during a game. The game administrator is to be informed of the problem the night of the game. Written reports from each official involved are to be submitted to the UIL office within the next two working days following the game or match.

The UIL report should be filed electronically using the UIL Incident Reporting Form. If the incident occurs during a TAPPS contest, use the TAPPS Incident Reporting Form.

Each official involved must also notify the TASO-Soccer San Antonio D&P Chair immediately and file a written report with the Chair within 24 hours of the incident. The report may be submitted by mail, FAX or email.
